VAT Exemption Form

We can supply certain products free from VAT tax if you suffer from certain illnesses, and you are using that product for that illness. We will show links on the products that you can buy without paying VAT. Please order as usual, and pay VAT with your order, so we can get the goods to you as quickly as possible. Then print out this form, fill it in, and post it to our address, at the bottom of the form, and we will refund your VAT when we receive your form. We will keep these forms on file, so that you do not need to send one with any future orders, but please e-mail us each time to claim your VAT free sale, as we do not check this ourselves.

Warning: Section 72(3) of the VAT Act 1994 provides for severe penalties for anyone who makes use of a document which they know to be false for the purpose of obtaining VAT relief.
